The information above is accurate, and the email named is the person who will hold the portal login. The administrative fee is one-time and non-refundable once the portal is provisioned. One portal per company. The one-time access code works exactly once. After it is used, access is by the password I create — Axiom never sees or stores that password.

STEP 1

Application & fee

You submit this form and pay the one-time $100 administrative fee. Nothing is provisioned until payment clears — nobody gets a portal without it.

STEP 2

One-time code

Your portal is created and a single-use access code is issued to the email on the application. It works once, for one person, and then it is dead.

STEP 3

Your password

The code signs you in; you immediately create the password only you know. From then on it is your email and your password — your portal, paid for and yours.
